Annual Parent Involvement Form (For Awardees Only)

As a recipient of one of the O.T.C. Memorial Scholarships, you are required to submit this Parent Involvement Form along with your annual student transcripts and achievement resume in order to receive your 2nd through 4th installments.  Funding will NOT be released without the required documents mentioned above.
